Returning Student
Thank you for continuing to choose Lockhurst Drive as your school of choice in the fall.
The following items must be submitted before or at the Back to School Social :
- Student Emergency Information Form
- Student Residency Questionnaire (If none apply please complete and select NO at the bottom of page)
- Migrant Education Family Questionnaire (Only if changed from original enrollment)
- American Indian-Alaskan Native Letter Questionnaire (Only if Applicable)
- Title III Form (Only if Applicable)
- Responsible Use Policy (All 3 pages required) (Even if NO needs to be completed and turned in)
- Publicity Authorization and Release (Required even if NO)
Change of address
- Verification of Residence - current utility bill in your name - (Utility Service Bill (Electric, Gas or Water), Property Taxes, Rental/Lease Agreement, Official Government Mail (CalWorks or Social Security)
Please be sure to inform the office if your address changes at any time during the school year. You will need to bring in verification of your new address (Utility Service Bill (Electric, Gas or Water), Property Taxes, Rental/Lease agreement, Official Government Mail (CalWorks or Social Security) and a new emergency card completed.
Parent/Student Handbook
Please take the time to read and review the LAUSD Parent/Student Handbook 2019 - 2020 with your child. Parent/Student Handbooks for 2019 - 2020 will be sent home the first week of school. Please review and sign all required forms. Signatures are required on the Parent/Student Acknowledgement Form, Information Release Form, and the Annual Pesticide Use form. Return them to your child's teacher.
