New Student

All new students enrolling into Lockhurst must have either accepted through Unified Enrollment or have verified you are living within our school boundaries.

Once either of the above steps have been confirmed please review the required forms listed below to start enrollment.

If you live with in our school boundaries or you have accepted a lottery spot you can enroll on line through Enroll.LAUSD.net

The following items must be submitted at the time of registration:

• Student Enrollment Form
• Student Emergency Information Form
• Doctor Health Card (Examination by private physician)
• Temporary/Student Health Card (Permanent Health History Form)
• Immunization Records (Please refer to the Parent’s Guide to Immunizations)
• Oral Health Assessment Letter/Waiver Request Form (only for Kindergarten or 1st grade entry by May 2019)
• Student Housing Questionnaire (If none apply please complete and select NO at the bottom of page)
• Migrant Education Family Questionnaire (Even if NO needs to be completed and turned in)
• American Indian-Alaskan Native Letter Questionnaire (Only if Applicable)
• Title III Form (Only if Applicable)
• Responsible Use Policy (All 3 pages required) (Even if NO needs to be completed and turned in)
• Publicity Authorization and Release (Required even if NO)
• Copy of Parent ID
• Birth Certificate Verification of Birth – Legal birth certificate (not hospital certificate)
• Verification of Residence - current utility bill in your name - (Utility Service Bill (Electric, Gas or Water), Property Taxes, Rental/Lease Agreement, Official Government Mail (CalWorks or Social Security)
